Histological Differences of Surrounding Mucosa according to Helicobacter pylori Infection and Preexisting Adenoma in Early Gastric Cancer

Abstract
Background/Aims: Helicobacter pylori (HP), atrophy, intestinal metaplasia (IM) and gastric adenoma are known to be the risk factors of gastric cancer. The aim of this study was to find histological risk factors by comparing grades of atrophy and IM of background mucosa according to HP infection and preexisting adenoma (PA).

Methods: We investigated 670 patients who had undergone endoscopic submucosal dissection due to gastric adenoma or early gastric cancer (EGC) in Seoul National University Hospital between August 2005 and November 2007. They were divided into 6 groups according to status of tumor, HP infection and preexisting adenoma. The grades of atrophy and IM at midbody and antrum were evaluated by updated Sydney grading system.

Results: Age and mean tumor size were larger in PA (£«) EGC than the other group. Mean grades of atrophy and IM in antrum and body were higher in PA (£«) EGC group than in PA (?) EGC group regardless of HP infection. In addition, mean grades of corporeal atrophy and IM were higher PA (£«) EGC group than in adenoma group.

Conclusions: Atrophy and IM of surrounding mucosa were more severe in gastric cancer with preexisting adenoma than in that without preexisting adenoma.
